Follow-up appointments monitor patient progress and evaluate the effectiveness of your treatment plan initiated at the first visit. If you are taking psychotropic medications, we will monitor current medication regimens, assess for side effects, and adjust dosing of medications as needed to achieve desired outcomes. If you are not taking medication, we will discuss the current treatment plan and modify as needed. The patient and clinician will collaborate to determine the length of appointment needed. This will depend on the complexity of the visit and the needs of the patient. The goal is to assess effectiveness, minimize side effects, and optimize the patient’s well-being.

Follow Up Visit After a Mental Illness Hospital Stay

If you’ve been hospitalized for a mental health concern, it’s crucial to follow-up with a mental health provider within seven days of being discharged. The goal of hospitalization during a mental health crisis is to decrease the risk of the individual harming themself or someone else. During hospitalization, the patient will be monitored while transitioning from an acute unhealthy, potentially dangerous situation to a more manageable condition. While stabilization is important, it is only the beginning of care. Ongoing treatment after a psychiatric hospitalization is vital to maintain stability, enhance daily functioning, improve patient outcomes, prevent relapses, and reduce the rate of future hospitalizations.
